MEMBER AGREEMENT ADDENDUM
Please review and complete the following membership addendum with FITNESS:1440- Lebanon, to resume using facilities during re-opening Oregon Phase 1 and 2
Dear Valued FITNESS:1440 Member,
As we begin to re-open, we are committed to providing our team and members with a clean and safe environment. In doing so, we need your help. Below are a few guidelines that we ask you to accept and follow when you visit the club. Please review each one, and if you agree to the guidelines, click “I ACCEPT” at the bottom of this form. We appreciate your cooperation in keeping our TEAM1440 community as healthy as possible.
For good and valuable consideration, the parties agree as follows:
- HEALTH & SAFETY
- I confirm I do not have any symptoms related to and have not been knowingly exposed to COVID-19.
- Have you had a new or worsening cough?
- Have you had a fever?
- Have you had shortness of breath?
- Have you been in close contact with anyone with these symptoms or anyone who has been diagnosed with COVID-19 in the past 14 days?
- If the above status changes, I agree not to visit the club.
- FACE COVERING
- We strongly recommend members wear masks while in the club.
- SOCIAL DISTANCING
- I agree to keep 6 feet between myself and other members while in the club.
- CLEANLINESS
- I agree to disinfect all equipment after use and will not enter closed off areas.
- I agree to keep my hands washed/sanitized while at the club.
FITNESS:1440 staff will be monitoring the gym floor – helping to clean, discourage proximity between members and encouraging members to wipe down equipment. If a FITNESS:1440 staff member or member asks you to do one of the above, we ask that you please cooperate and do your part. Thank you in advance, we cannot wait to see you in the club!
WAIVER
Considering the COVID-19 pandemic, I reaffirm the Waiver which I executed on my Member Agreement and assume the risk of use of the FITNESS:1440 facilities.
RELEASE OF LIABILITY
I accept responsibility for my use of any and all apparatus, appliance, facility, privilege, or service whatsoever, owned and operated at FITNESS:1440 at my own risk, and shall hold this club, its shareholders, directors, employers representatives and agents harmless from any and all loss, claim, injury, damage, or liability sustained or incurred by me resulting therefrom.
